TV Services Video Checkout Policy

  1. Professors, lecturers, and teaching assistants (TAs) can check out videos for playback in their course.
  2. Professors, lecturers, and teaching assistants (TAs) can check out up to five tapes for a period of seven days.
  3. Professors, lecturers, and teaching assistants (TAs) sign an agreement form, when picking up tapes, that has this policy written at the bottom of the form.
  4. Videotapes/DVDs are renewable by clients through walk-in, call-in ext. 3518, or e-mailing fvo@id.ucsb.edu. Renewals are for a one-week period and can be renewed throughout the quarter.
  5. All tapes are due back at Television Services/Film and Video ordering by the end of the quarter.
  6. Returnees with overdue tapes/DVDs are fined a $3.00 fee per tape, per day late.
  7. Late fees are charged to the faculty member's department, and it is the faculty member's responsibility to inform their department of this charge.
  8. Television Services will attempt to contact the Faculty member as a courtesy regarding a past due tape, but please keep in mind that it is not our responsibility to remind faculty when tapes are due.
  9. Lost or damaged tapes will be replaced by Television Services but will be charged to the Faculty member's department.
  10. When billing for an overdue tape, Television Services will contact the faculty member and their MSO or financial personnel, within their department, to request a recharge number and to inform them of the reason for this billing.
